The parent and student will fill out all portions of the application and submit to CHA. This section can be done online or completed on paper. Click the button below to get started online.
After the written section there are two recommendations for each applicant. One recommendation form should be filled out by a teacher who has had this student for at least one marking period. The second recommendation form is to be completed by the principal/counselor/tutor/coach or someone that has a good understanding of the student outside his/her immediate classroom. The recommendation forms must be accompanied by the student’s grade school transcripts/report card. The applicant merely needs to fill out the Request for Records form and turn that into their student’s current school.
2. ENTRANCE EXAM
CHA will administer the Sholastic Testing Service’s High School Placement Test (HSPT) on various Saturdays throughout the year for incoming students. (If extenuating circumstances prevent attendance, alternative times can be arranged). Once the admissions office has received the completed admission application and student questionnaire, the student should sign up for a testing date. All applicants are required to take this test in order to be considered further for acceptance. If the student has already taken the HSPT at another school, he/she can have those scores faxed/mailed to Chicago Hope.
3. FAMILY INTERVIEW
After evaluating the completed application and entrance exam results, the admissions coordinator will contact the prospective student about acceptance or schedule a family interview. An interview is not guaranteed for every applicant. In the interview, the admissions coordinator is looking for the student’s leadership potential, personal desire to attend CHA, and willingness to work hard once admitted.
The admissions coordinator will also question the parents to determine the level of family involvement and support. Prospective families will know by mail within two weeks of the interview, if not sooner.
